New to Germany?
How to get your health insurance.

Starting a job in Germany? Then you are usually covered by public (statutory) health insurance – and you choose your health insurer (Krankenkasse) yourself. Basic benefits are the same at every insurer, the contribution is not. Here you see what applies to you, what you need and how to join online. Free of charge.

Key facts at a glance

Health insurance in Germany: what applies to you

With a job, you are usually insured compulsorily

If you are employed and earn less than the compulsory insurance threshold – 2026: €77,400 gross a year (€6,450 a month) – you are automatically covered by public health insurance. For 2027, €84,150 has been adopted by the government; the Bundesrat still has to approve it.

You choose the health insurer

You decide which insurer. Tell your employer within two weeks of your first working day at the latest – otherwise they register you with an insurer of their choice. Your cover starts on your first working day.

What it costs

14.6% of your gross salary (up to the contribution ceiling) plus your insurer’s additional contribution – in 2026 between 2.18% and 4.39%. Your employer pays half. Long-term care insurance comes on top. With €4,000 gross and a 2.9% additional contribution, you pay €350 a month for health insurance.

Family often covered free of charge

Spouses or civil partners and children who live in Germany and have little income of their own (2026: up to €565 a month, up to €603 with a mini-job) are insured free of charge. From 2028 you pay a surcharge for a co-insured partner; children stay free.

Earning more? Join voluntarily

If your salary is above the threshold, you can join public health insurance voluntarily within three months of starting your first job in Germany. Your employer then pays a contribution subsidy of the same amount as for compulsorily insured employees. The alternative is private health insurance – switching back later is difficult.

Same basic benefits everywhere

Doctors, hospital, medicines: statutory benefits are the same at all insurers, with no health check and no waiting period. Differences lie in the additional contribution, extras such as dental cleaning, and service.

Step by step

From visa to health insurance card

1

Visa: proof until your job starts

Public health insurance only starts on your first working day. The insurer can usually confirm in writing that you will be covered from then on. For the time before, the embassy usually requires travel or incoming health insurance. Your embassy will tell you exactly what applies.

2

Choose an insurer and apply

Click “Apply” in the list below. In the application, answer “No” to the question whether you are currently covered by statutory health insurance if you have never been insured in Germany before. Occupational group: employee.

3

Tell your employer

Tell your employer which insurer you are with. They receive the membership certificate electronically. You don’t need a social security number beforehand – it is issued when you are first registered.

4

Get your health insurance card

The insurer sends your health insurance card by post; you need a photo for it. Until it arrives, the insurer can give you provisional proof for doctor visits on request.

Special cases

  • Posted to Germany by an employer in another EU country? If you are only temporarily posted to Germany, you usually stay insured in your home country – proven by the A1 certificate.
  • Self-employed or freelancer? Then you are not compulsorily insured. Public insurance is only possible voluntarily with a previous insurance period – statutory insurance in another EU country can count (form S041). You must apply within three months of your previous insurance ending. Otherwise, private health insurance remains.
  • Only a mini-job? A mini-job on its own does not make you publicly insured. You then need other cover – ask us.
  • Coming to Germany to study? Different rules apply – see Health insurance for international students.

Frequently asked questions

Questions about health insurance for newcomers

Do I have to have health insurance in Germany?
Yes. Everyone who lives in Germany must have health insurance – public or private. As an employee below the compulsory insurance threshold, you are automatically covered by public health insurance as soon as you start working.
When does my health insurance start?
On your first working day. For the time before – for example for the visa or your first days after arrival – you need other cover, such as travel or incoming health insurance.
Which health insurer is best for newcomers?
There is no single best insurer. Basic benefits are the same everywhere. Differences lie in the additional contribution, extras and service – some insurers also offer advice and apps in English. If you want to compare, use the calculator on this page.
Public or private – which is better?
If you earn below the compulsory insurance threshold, you have no choice: you are publicly insured. Above it, you can choose. Public insurance bases the contribution on your income and often covers family members free of charge. Private insurance calculates by age, health and tariff. Once you are private, you can only return under certain conditions.
Can I insure my family?
Yes, if they live in Germany and have little income of their own (2026: up to €565 a month, up to €603 with a mini-job). Spouses or civil partners and children are then insured free of charge. From 2028 a surcharge applies for a co-insured partner; children stay free. Exception: if you are married and your partner is not publicly insured, earns above the compulsory insurance threshold and more than you, the children are not insured free of charge.
What do I need for the application?
Passport or ID card, your address in Germany, details of your employer and – if you have one – your social security number. For the health insurance card, the insurer will ask for a photo.
Can I switch insurers later?
Yes. After 12 months of membership you can switch at any time. If your insurer raises its additional contribution, you have a special right of termination and can leave earlier.
What does your help cost?
Nothing for you. The list, comparison and application are free of charge. If a membership is arranged through us, we receive a fee from the health insurer – not from you.
