Taking out statutory health insurance for the first time?
Choose your health insurer yourself.

First job, studies, new to Germany or coming back from private health insurance: in most cases you are free to choose your health insurer – and the additional contribution differs by around 2.2 percentage points between the cheapest and the most expensive insurer. Here you see the insurers with their contribution and apply directly online. Free of charge, no comparison needed first.

Which applies to you?

Six typical situations – and what to enter in the application

In the online application, our partner VGK24 first asks: are you currently covered by statutory health insurance? Important: if you were covered through your parents or spouse (family insurance), you are covered by statutory insurance – choose “Yes” and enter their insurer.

Apprenticeship or first job

If you start a paid apprenticeship or a job above the mini-job limit, your family insurance through your parents ends. From then on you are compulsorily insured yourself – and choose your insurer freely. A mini-job on its own does not change this.

In the application: “Yes”, plus your parents’ insurer. Occupational group apprentice or employee.

More for career starters →

Studying from 25: your own health insurance

Family insurance ends at 25 (up to twelve months later after military or voluntary service). You then usually become compulsorily insured yourself as a student – until 30 at the latest – and choose your insurer freely. You pay the contribution on your own, so a cheaper insurer pays off twice.

In the application: “Yes”, plus your previous insurer. Occupational group student.

Costs and deadlines for students →

New to Germany: health insurance for your first job

If you are employed here and earn below the compulsory insurance threshold (2026: €77,400 a year), you are compulsorily covered by statutory health insurance. You choose the insurer yourself and tell your employer within two weeks – otherwise they register you with an insurer of their choice. If you earn more, you can join voluntarily within three months of starting your first job in Germany. You don’t need a social security number beforehand; it is issued when you are first registered.

In the application: “No”. Occupational group employee.

International students

To enrol, your university needs proof of your health insurance – the insurer sends it electronically. If you have statutory insurance in another EU country, your European Health Insurance Card may be enough. Compulsory student insurance only starts with enrolment and applies until 30 at the latest – for a language course or Studienkolleg you usually need private insurance.

In the application: “No”. Occupational group student.

From private back to statutory health insurance

Returning is mainly possible once you become subject to compulsory insurance again – for example because your salary falls below the compulsory insurance threshold (2026: €77,400 a year) or you receive unemployment benefit. Citizen’s income (Bürgergeld) is not enough. From 55, the way back is usually closed.

In the application: “No”. Occupational group according to your work.

All ways back and the 2027 threshold →

Family insurance ends

For example after a divorce, when your own income rises above the limit for family insurance, or when your partner switches to private insurance. If you start a job or studies at the same time, you choose your insurer freely. Otherwise you automatically become a voluntary member of your previous insurer – you can still switch.

In the application: “Yes”, plus your previous insurer.

Special cases where you cannot choose freely

  • Self-employed as your main occupation and never had statutory insurance? Then you usually cannot join a statutory insurer. Take a look at private health insurance.
  • Back from abroad without a new job? Then you are usually insured again with the insurer you were last with. With a new job, you choose freely.
  • Family insurance ends without a job or studies? Then it automatically continues as a voluntary membership with your previous insurer. You can switch by joining another insurer within three months or later with the notice period – see Family insurance ends.
  • Civil servant? Then you are usually privately insured, alongside the state allowance (Beihilfe).

Additional contribution 2026 according to the list of health insurers published by the GKV-Spitzenverband, retrieved on 1 November 2026. Listed are the 53 insurers we arrange via our partner VGK24 (out of 71 insurers open to the public) – sorted by additional contribution, not by remuneration. You will find all insurers in the full list. Benefits differ – compare contribution and benefits before you decide.

How the application works

Your own insurer in 4 steps

1

Choose your insurer

Click “Apply” in the list above. The insurer is already filled in.

2

Three short questions

Your occupational group, whether you are currently covered by statutory insurance and – if so – with which insurer.

3

Fill in the online application

Personal details, address, insurance details and occupation. Already with an insurer? The new insurer handles the termination for you.

4

Confirmation and card

The membership certificate goes electronically to your employer or university. Your health insurance card arrives by post – the insurer will ask you for a photo.

Online application & comparison

Prefer to compare first?

The calculator shows contributions and benefits for your income. If you are not yet covered by statutory insurance, choose “Not insured” as your insurance status – family insurance counts as “Statutorily insured”. If you chose an insurer above, its online application appears here.

Note: The comparison and the application are free of charge. Legal information can be found in the legal notice, the privacy policy and the initial information.

Remuneration notice: We act as an insurance broker and may receive remuneration if joining or switching to a health insurer comes about through our brokerage. This is paid by the receiving health insurer – there are no additional costs for you.

Ranking notice: The order of the health insurers shown may be influenced by the amount of remuneration. The comparison does not claim to include every health insurer available on the market.

Frequently asked questions

Questions about your first health insurer

Can I choose my health insurer myself?
In most cases, yes. Anyone who becomes subject to compulsory insurance – for example through an apprenticeship, a job, studies or unemployment benefit – chooses their insurer freely. Insurers open nationwide must accept you, regional insurers if you live or work in their area – without a health check and without a waiting period. Exceptions: company-based insurers only accept employees of certain companies. If your cover automatically continues as a voluntary membership after family insurance ends, you first stay with your previous insurer – you can switch by joining another insurer within three months or later with the notice period. And if you return after a period without insurance, you usually become a member of your last insurer again.
I was insured through my parents – am I “covered by statutory insurance”?
Yes. Family insurance is statutory insurance. In the application, choose “Yes” and enter your parents’ insurer. “No” is for everyone who was previously insured privately, abroad or not at all.
What happens if I don’t choose an insurer?
As an employee, you must tell your employer which insurer you are with within two weeks of becoming subject to compulsory insurance – usually your first working day. If you don’t, your employer registers you – with the insurer you were last insured with, otherwise with an insurer of their choice. That is not necessarily the cheapest.
Why aren’t all insurers listed here?
We offer the online application for the insurers we arrange via our partner VGK24. Mainly company-based insurers and some company health insurance funds (BKK) are missing here. You will find all insurers with their additional contribution in the full list – for the others, you apply for membership directly with the insurer.
Which documents do I need?
ID card or passport, your address and – if you have one – your social security number. For the health insurance card, the insurer will ask for a photo. If you are coming from another insurance, you often enclose proof of your previous cover.
Can I switch insurers later?
Yes. After 12 months of membership you can switch at any time. If your insurer raises its additional contribution, you have a special right of termination and can leave earlier.
What does it cost?
Nothing for you. The list, comparison and application are free of charge. If a membership is arranged through us, we receive a fee from the health insurer – not from you.
