Health insurance 2027
What changes – and what you can do now.

From January you pay more yourself for prescriptions, in hospital and for dentures, no insurer pays for homeopathy any more, and the contribution thresholds rise. What stays the same: each insurer sets its own additional contribution – and that's where you can save.

Key facts at a glance

The 2027 changes at a glance

Copayments rise by half

For medicines you still pay 10% of the price, but at least €7.50 instead of €5 and at most €15 instead of €10. In hospital €15 instead of €10 per day; for physiotherapy and similar treatments €15 instead of €10 per prescription plus 10% of the costs.

Lower subsidy for dentures

The fixed subsidy falls from 60% to 50% of standard care, with a bonus booklet to 60% (after five years) or 65% (after ten years). For treatment plans your insurer approves in 2026, the old rates still apply.

Homeopathy only at your own expense

From 2027, insurers no longer pay for homeopathic and anthroposophic medicines and treatments at all – not even as an extra benefit. Herbal medicines are not affected.

Higher contribution thresholds

The contribution assessment ceiling rises to €6,375 a month, the compulsory insurance threshold to €84,150 a year – with a grandfathering threshold of €80,550 for people already privately insured. Higher earners pay more. The Bundesrat still has to approve.

Additional contribution: average 2.9%

The official average stays at 2.9% in 2027. What you pay is decided by your insurer – most set their rate in November and December. The range already runs from 2.18% to 4.39%.

Sick pay: new rules

Self-employed people who opt into sick pay by declaration will have to wait three months from 2027 – unless they declare it within 14 days of their membership starting. If your job ends while you are ill, you then receive 60% of your net pay, 67% with a child.

Timeline

What applies when

Most changes come from the GKV contribution rate stabilisation act (published in July 2026), the thresholds from the 2027 calculation values ordinance. Everything applies equally at all statutory insurers – except the additional contribution.

  1. Since 30 July 2026

    • Your insurer no longer has to write to you when it raises its additional contribution. The Bundestag wants to reintroduce the obligation; the Bundesrat still has to discuss it.
    • Consumable medical aids such as incontinence products: copayment of at most €15 instead of €10 a month.
  2. 1 January 2027

    • Higher copayments for medicines, medical aids, hospital stays, therapeutic treatments and home nursing care.
    • Lower fixed subsidy for dentures: 50% instead of 60%, with a bonus booklet 60% or 65%.
    • No more homeopathy or anthroposophic medicine at the insurer's expense.
    • Contribution assessment ceiling €6,375 a month, compulsory insurance threshold €84,150 a year (Bundesrat approval pending).
    • New additional contributions from the insurers. If yours goes up on 1 January, you can give notice until 31 January 2027.
    • Sick pay: three-month waiting period after the declaration for self-employed people; 60% or 67% of net pay if your job ends while you are ill.
    • Planned, not yet decided: higher long-term care surcharge for childless people (0.9 instead of 0.6 percentage points).
  3. 1 January 2028

    • A surcharge of 2.5 percentage points if your spouse or civil partner is covered through you free of charge – with exemptions, for example for parents of children under 12.
    • Partial sick notes (25, 50 or 75% of working hours), only with your consent and your employer's – by law from 1 January 2028. The Bundestag has decided to postpone the start to 1 July 2028; the Bundesrat still has to approve.
    • Before a planned hip or spinal operation you need a second opinion, otherwise the insurer won't pay. More procedures follow from 2029.
  4. 1 July 2028

    • The partner surcharge also applies to the state pension.

What you can still do in 2026

Dentures: get the plan approved in 2026

What counts is the insurer's approval, not the treatment date. Have the treatment and cost plan prepared in time and submit it. An approved plan is usually valid for six months.

Check the additional contribution in December

Because insurers no longer have to write to you, check yourself what yours charges from January. If it raises its rate, you can give notice until 31 January 2027 – even without 12 months of membership. If you've been a member for 12 months, you can switch at any time.

Keep your receipts

You never have to pay more than 2% of your gross annual income in copayments, 1% with a chronic illness. With higher copayments, more people will reach this limit. Keep the receipts and apply to your insurer for an exemption.

Self-employed: choose sick pay in time

From 2027 the sick pay declaration usually only takes effect after three months. If you want sick pay and haven't chosen it yet, talk to your insurer about submitting the declaration in 2026.

Privately insured: check your 2027 salary

If you earn no more than €80,550 in 2027, you become publicly insured again. You can apply for an exemption until 31 March 2027 – it cannot be revoked.

Partner covered through you? Plan early

From 2028, family insurance for your partner costs a surcharge – around €100 a month on €4,000 gross. Check whether an exemption applies – or whether a job with its own statutory insurance is worthwhile for your partner (a mini-job alone isn't enough).

What changes at health insurers in 2027?
From 1 January 2027 copayments rise by around half, the fixed subsidy for dentures falls from 60% to 50%, homeopathy is no longer covered, and the contribution assessment ceiling rises to €6,375 a month. Insurers also set new additional contributions.
How high is the additional contribution in 2027?
The official average in 2027 is 2.9%, as in 2026. It is a reference value, not a price: each insurer sets its own additional contribution, mostly in November and December. In 2026 the range runs from 2.18% to 4.39%.
How much does a prescription cost from 2027?
10% of the price, at least €7.50 and at most €15 – never more than the medicine costs. A €30 medicine costs you €7.50 instead of €5, a €200 one €15 instead of €10.

Can I switch if my insurer becomes more expensive in 2027?
Yes. If your insurer raises its additional contribution on 1 January 2027, you can give notice until 31 January 2027, even without 12 months of membership. Simply apply to the new insurer; it handles the termination.

It is planned that childless people pay a surcharge of 0.9 instead of 0.6 percentage points, i.e. 4.5% instead of 4.2%. For parents it is to stay at 3.6%. This has not been decided yet. The care contribution is the same at all insurers.
What does your help cost?
Nothing for you. Comparison and application are free. If a membership comes about through us, we receive remuneration from the health insurer – not from you.
