How much does your gym cost after switching?

All statutory health insurers charge their own additional contribution on top of the uniform rate of 14.6%. You pay half of it yourself – your employer pays the other half. If you switch from an expensive to a cheap insurer, you save every month. Move the sliders to your own figures and see how much your gym effectively still costs after switching.

Example calculation – the figures are based on a realistic switching benefit with the best choice of insurer (1.5 percentage points). Your actual savings depend on your current insurer and the one you newly choose.

Your contribution savings alone already cover a large part of your gym fee. On top of that, many insurers offer sports bonuses and subsidies for prevention courses – as a separate benefit.

Example calculation based on a contribution range of 1.5 percentage points – a realistic switching benefit with the best choice of insurer. It takes into account the employee's share (50% of the contribution difference). In 2026 the range between the cheapest and the most expensive insurer is around 2.2 percentage points (2.18% to 4.39%, source: list of health insurers published by the GKV-Spitzenverband, the national association of statutory health insurers). Not every insurer can be chosen in every federal state – among insurers available nationwide the range is around 1.9 percentage points. Actual savings depend on the insurer you choose. Your health insurer does not subsidise your gym directly – what is shown is the effect of the contribution savings. Possible sports bonuses and subsidies for prevention courses come on top separately, depending on the insurer.

Bonus programme
Gym & sport are rewarded.

Many insurers reward regular sport with cash bonuses or non-cash rewards – through their bonus programme under §65a SGB V. Amounts and conditions vary considerably.

Prevention courses
Back training, yoga, Pilates & more.

Under §20 SGB V, certified prevention courses are partly subsidised – usually several per year. Each insurer decides individually which courses qualify and how much it pays.

Contribution
More take-home pay – budget for training.

In 2026 there is a range of around 2.2 percentage points in the additional contribution between the cheapest and the most expensive insurer. That gives you room to manoeuvre – without any change to your gross pay or your training.

Background

Why comparing pays off for people who train

Many statutory health insurers subsidise gym memberships, prevention courses and sporting activities – but not all to the same extent. Which insurer gets the most out of it for you depends on how you train, the benefits you want and your income.

Then there's the difference in contributions: while the general contribution rate is identical at all insurers at 14.6%, the individual additional contribution varies considerably – by several hundred euros a year. If you optimise both together, you win twice: a lower contribution & support for your training. In just a few minutes you'll see which insurer suits your situation. No sign-up, no obligation.

Background

How your insurer's sports support works.

Bonus programme – rewards for sport

Under §65a SGB V, statutory health insurers may offer their own bonus programmes – and they increasingly do. If you do sport regularly, you can have this activity counted: through an existing gym membership, a sports club membership, by taking part in fun runs or sports badge tests (Sportabzeichen), and in some cases also through step-counter records via the insurer's app.

In return, the insurer rewards the activity – either with a cash bonus, with vouchers or with non-cash rewards (e.g. sports equipment, wearables). The amount is capped and varies considerably between insurers: some pay a fixed annual bonus, others use a points system that combines various health activities.

What to look out for: How high is the maximum annual bonus? Which proof is accepted (gym receipt, club confirmation, app data)? Is there a minimum level of activity (e.g. 50 gym visits per year)? And above all: is the bonus paid out in cash or granted only as a non-cash reward?

Prevention courses under §20 SGB V

§20 SGB V obliges statutory health insurers to provide benefits for primary prevention and health promotion. In practice, this means that certain certified courses on exercise, nutrition, stress management and addiction prevention are partly subsidised. Typical examples in the exercise category: back training, Pilates, yoga, aqua fitness, running groups or functional training programmes.

As a rule, courses certified by the Zentrale Prüfstelle Prävention (ZPP, the central testing body for prevention) qualify. You can usually recognise this status by the note “ZPP-anerkannt” (ZPP-approved) or the corresponding logo. Purely online courses can also qualify, provided they are ZPP-certified.

Each insurer sets its own subsidy: proportional reimbursements are common (e.g. 80 or 100% of the course fee) up to a maximum amount per course and a maximum number per year. If you use courses regularly, you should compare carefully – the differences between insurers can amount to several hundred euros a year.

Why comparing additional contributions pays off twice for athletes

The statutory contribution rate of 14.6% is identical at all health insurers. What differs is the individual additional contribution, which each insurer sets itself. In 2026 the range between the cheapest and the most expensive insurers spans around 2.2 percentage points. As this share is paid half by the employee and half by the employer, the difference is reflected directly in your net pay – without any change to your gross pay.

For athletes, this creates a double lever: first, the contribution can be lowered directly, which gives you more room for your gym membership, equipment or competition fees. Second, there's the bonus effect: if you train regularly anyway, many insurers let you turn this activity into extra money as well. In total, active members can gain several hundred euros a year – depending on income, on which insurer is chosen and on which benefits are actually used.

Important: after 12 months of membership you can switch insurer at any time. If your current insurer raises its additional contribution, you also have a special right of termination. The switching process is fully online, takes a few minutes, and since 2021 the membership certificate has been sent to your employer electronically and automatically – paper certificates are no longer needed.

Frequently asked questions

FAQs on sport & health insurance

Which health insurer pays the most towards the gym?
There's no general answer – each insurer's bonus programme is structured differently. Some insurers pay a fixed annual bonus, others use points systems in which a gym membership is one of several activities. In the comparison calculator you can filter specifically by bonus programme and compare the conditions side by side.
How do prevention courses under §20 SGB V work?
Health insurers are legally obliged to provide health promotion benefits. In practice, they partly subsidise certified courses (e.g. back, yoga, Pilates, functional training, nutrition, stress management). Only courses certified by the Zentrale Prüfstelle Prävention (ZPP) qualify. Each insurer sets the amount and the number of subsidised courses per year itself.
Do I need a minimum amount of training for the bonus?
That depends on the insurer. Some require a certain minimum number of gym visits per year (often documented via training records or check-in stamps), others reward proof of an active membership alone. You'll find the exact conditions in your insurer's bonus programme rules.
Are sports clubs or online fitness subsidised too?
Yes, most bonus programmes also accept sports club memberships. With online fitness offers, it depends: purely commercial online training apps are generally not rewarded through the bonus programme. Online prevention courses, on the other hand, can qualify under §20 SGB V – provided they are ZPP-certified.
What do I need to submit to receive the bonus?
Typical proof includes a current gym or club confirmation, a receipt for a paid course, or for some insurers data from the insurer's app (e.g. steps, workouts). You'll find the exact list of accepted proof and the submission procedure (online, app, post) in the bonus conditions of the insurer concerned.
Are the basic benefits the same at all insurers?
Yes. The statutory basic benefits – doctor's visits, medication, hospital stays, preventive check-ups – are identical at all statutory health insurers and governed by the German Social Code (Sozialgesetzbuch). Differences exist only in voluntary extra benefits such as bonus programmes, prevention courses, osteopathy, app quality or customer service.
How long am I tied to the insurer I choose?
At least 12 months. After that you can switch at any time, with a notice period of two months to the end of the month. If your insurer raises its additional contribution, you also have a special right of termination and can switch earlier.
What does the comparison cost?
Nothing. The comparison is 100% free and non-binding. Using the calculator creates no obligation whatsoever – and even if you switch afterwards, there are no costs for you.
